Answer James, you may have lost the cold water solenoid on your water valve. On the cold/cold cycles it is not calling for any hot water. On cold cycles, if there is no cold water coming through the valve, it will not fill at all. On warm and hot, you may be getting hot water without any cold adding to the water mixture.
